Online-Submitted Electronic Forms

Add/Drop Enrollment Request Form

The following form is made available by the University Registrar’s Office and will remain active throughout the Spring 2025 regular session add/drop period.

Please be aware the form is not a substitute for time conflicts or credit overload requests which you will find in the Additional Forms section.

Email-Attached Forms

Please note: Incomplete forms will not be accepted and will be sent back to you.

Instructions for Submitting Email-Attached Forms

  • You will need to download these forms to edit them, to save to your computer, or to print.
  • Please try to obtain actual signatures of approval on the editable PDF forms. If you cannot obtain actual signatures, please obtain approval via email from the appropriate approver. In those cases, please indicate “see attached email” in the spaces for signatures and be sure to submit your email approval as proof.
  • All necessary signatures, approvals, and supporting documentation (i.e. syllabi for the Transfer Credit Application) for the particular form must be included and sent all together as attachments to your email at the time of your submission.
